- Perform health care institution and physician office hospice education activities.
- Coordinate patient services.
- Coordinate community hospice education activities.
- Coordinate with health care institution and physician education activities.
- Interact with clinical leadership
- Requires a bachelor's degree or RN, MSW. Prefer two years' experience working with home health/hospice, hospitals, physicians, HMO's, outpatient services or social services concerning public relations or office procedures.
- Knowledge of Medicare/Medicaid hospice regulations preferred. If licensed, successful completion from an approved school of nursing or college education.
- Ability to coordinate the transfer of patients to hospice services, resulting in uninterrupted continued medical care.
- Must possess a strong knowledge of hospice services, comprehension of federal, state and ACHC guidelines.
- Effective communication skills (oral and written). Strong interpersonal and telephone skills.
