    Primary Purpose:

    We are looking for a skilled and compassionate Nurse Practitioner to join our Advantmed provider network. As a Nurse Practitioner, you will conduct in-home wellness risk adjustment assessments for Medicare and other populations. The main goal of this role is to evaluate the overall health of member beneficiaries to ensure precise risk adjustment coding, which ultimately leads to enhanced value-based care.

    NP Responsibilities:

    • Perform annual wellness visits and health assessments for a population with chronic conditions
    • Provide patient health education opportunities
    • Assist in closing quality care gaps such as screenings and labs
    • Collaborate effectively to improve care delivery
    • Commit a minimum of 30 hours per month

    Requirements

    NP Qualifications:

    • Must possess a valid and unrestricted NP License for the state of employment
    • Prior experience in in-home Risk Assessment is desirable
    • Preferably, have 3 years of patient care experience in primary/adult/geriatric care and experience with EMR
    • Being bilingual is an advantage
