    Job DescriptionThe Mobile Care Coordinator is responsible for picking up and delivering Hamakua-Kohala Health Center patients to and from their health care appointments. In doing so, the Mobile Care Coordinator will be responsible for the scheduling and coordination of services to maximize vehicle utilization and to meet the needs of the Health Center patients and staff. The Mobile Care Coordinator will work closely with, and communicate with the Clinical staff and will interface with the Clinical Services Manager in completing all tasks in the Representative Duties outlined below. The Mobile Care Coordinator may be called upon to provide information to the community regarding the Health Center and related services. REPRESENTATIVE DUTIES

    The following duties are typical for the Mobile Care Coordinator. Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.

    • Responsible for picking up and delivering Hamakua-Kohala Health Center patients to and from their health care appointments
    • Responsible for assisting with making patient specific transportation care plans and assisting with the implementation of the plan.
    • Assists patients (e.g. seating, restraints, special equipment, etc.) for the purpose of providing safe loading and unloading from vehicles including both emergency situations and normal transport.
    • Assists patient through the healthcare system by acting as a patient advocate and navigator.
    • Works in collaboration with providers and nursing staff, patients and their families to:
      1. Promote timely access to appropriate care
      2. Increase utilization of preventive care
      3. Reduce emergency room utilization and hospital readmission
      4. Connect patients to relevant community resources.
      5. Facilitate transition from emergency room and hospitalization to follow up appointment at Hamakua-Kohala Health.
    • Manages databases of assigned insurance carriers to identify existing care gaps and promote patient compliance.
    • Participates as part of the team for data collection, health outcomes reporting, clinical audits, identification of gaps in care, and programmatic evaluation related to Care Coordination.
    • Participates in Patient-Centered Medical Home quality improvement initiatives.
    • Demonstrate positive interpersonal relations with patients, their families, physicians, co-workers in a confidential manner.
    • Maintain a clear understanding of the delivery of patients to and from their destination.
    • Communicate all functions of job to supervisor on a timely manner.
    • Maintain quality of service during periods of increased or decreased patient load.
    • Embrace the philosophy of continuous quality improvement.
    • Apply problem solving and conflict resolution skills where needed.
    • Develop and maintain a schedule of service to best meet patient and Health Center needs.
    • Maintain a travel log, which records the number of patients transported and miles traveled daily.
    • Responsible for scheduling maintenance of vehicle- oil changes, car washes, etc. and keeping vehicle interior clean
    • Responsible for the upkeep of oil, water, fuel, etc. for safe operation of the vehicle.
    • Other duties as assigned by supervisor.

    EDUCATION, EXPERIENCE & QUALIFICATION GUIDELINES

    Any combination of education and experience that would likely provide the required knowledge and abilities is qualifying.

    • High School Graduate or GED Equivalent
    • Must have a current Hawaii Driver License
    • Experience working in a team environment as a contributing team member.
    • Clean Driver's Abstract- will be reviewed annually
    • Current CPR/ First Aid Certificate
    • Knowledgeable of the Hamakua – Kohala Areas
