    POSITION PURPOSE:

    Reporting to a Director of Clinical Services, the Wound Care RN delivers quality care to patients and performs skilled technical care in the patient's home, complying with nursing standards and all regulations.

    ORGANIZATION:

    This role reports to the Director of Clinical Services.

    Essential Functions:

    - Provides personalized nursing care to assigned patients following nursing standards and under the supervision of the Director of Clinical Services
    - Ensures safe and high-quality delivery of home health services to patients and their families/caregivers
    - Executes the care plan prescribed by the physician and Registered Nurse
    - Maintains accurate and timely documentation in line with the care plan
    - Educates patients, families, and caregivers on diagnosis and care plan
    - Promotes a safe working environment and uses proper work methods following company standards
    - Collaborates on home health services coordination with the Registered Nurse and clinical management
    - Guides patients in learning and performing self-care activities
    - Efficiently utilizes equipment and supplies
    - Documents and reports activities as necessary
    - Fulfills other assigned duties

    Knowledge & Experience:

    - Completed state-approved training program (e.g., diploma, certificate, or associate degree) AND an Associates Degree or Bachelor of Science in Nursing
    - Wound Care Certification completion with 1-2 years of experience
    - Possession of an RN license (NCLEX-PN exam passed)
    - BLS certification
    - 1-2 years of recent nursing experience
    - Skills: medical knowledge, patience, compassion, critical thinking, infection control, physical endurance, observation skills, safety awareness, communication, leadership, empathy

    MACHINES & EQUIPMENT:

    The incumbent must adeptly operate the current computer system, telephone system, and other office machines such as copiers.

    Physical Requirements:

    The position demands moderate to heavy physical exertion, including extensive driving and assisting patients with mobility activities. The employee must be capable of lifting/moving over 100 pounds and performing various physical tasks. Reasonable accommodations are available for individuals with disabilities.

    WORKING CONDITIONS:

    Work settings include offices, patient homes, exposure to diseases, and driving. Security and privacy training will be provided to safeguard the confidentiality and integrity of associate and customer data.
