    Practice Manager Belmont Cragin, Chicago, IL, US3 days ago Requisition ID: 3443Position SummaryThe Practice Manager is a member of the site leadership team who works to ensure the best environment for quality patient-centered care, while overseeing and coordinating all operations at the health center.

    The Practice Manager also serves as a liaison to providers and is accountable to/collaborates with the Health Center Medical Director (HCMD), with a direct reporting to the Director of Practice Management.

    Duties and ResponsibilitiesWorks directly with the Health Center Medical Director and providers to ensure a productive and efficient medical home.

    Trains assigned staff, as necessary, to maintain efficient, effective, patient-centered clinical operations.
    Participates in recruitment, interviewing, hiring, and evaluating site administrative and clinical staff.
    Creates and manages scheduling templates for all providers, in coordination with health center leadership.

    Supervises staff to ensure patient needs are addressed and met in a timely manner and that all services are documented in the electronic health record and/or practice management systems.

    Assists in development of, maintains knowledge of, and oversees adherence to, operational policies and procedures, as well as upholds and promotes human resources policies and procedures.

    Fosters a professional working relationship amongst all members of the health care teams.
    Supports, supervises and collaborates with all teams to create a patient-centered environment driven by a strong care coordination model.
    Assists team members to raise practice standards as put forward by the evolving model of patient-centered care.
    Assists in developing, implementing, and carrying out educational/in-service programs and quality improvement programs and activities.

    Works directly with Outreach and Enrollment Manager, Nurse(s), and Manager of Care Coordination to ensure that all staff are communicating effectively and working collaboratively in support of the patient-centered model of care.

    Conducts site and support staff meetings on a regular basis.
    Attends organizational meetings, as requested.
    Responsible for health center budget oversight/management and participates in budget preparation, in collaboration with HCMD and Finance.
    Participates in continuing education and professional growth.
    Reviews staff time cards for accuracy and submits in a timely manner.
    Provides primary oversight and management of VFC programs and purchased vaccines for health center.
    Coordinates and manages onsite audits as identified by payers or funders.
    Maintains inventory of clinical supplies, equipment, instrumentation, medications, and vaccines.
    Conducts/submits reports on EOC rounds monthly.
    Oversees site identification, documentation, resolution and reporting of occurrences and patient complaints.

    Adheres to/oversees the implementation of standards and policies of organizational privacy/security and compliance programs and complies with applicable law and regulations.

    This also includes reporting to Director of Practice Management, any unethical, fraudulent, or unlawful acts or practices.
    Performs front-line staff duties, when required.
    Other duties and responsibilities as assigned.

    Must be able to move around the clinic site 50% of the time.
    Constantly operates a computer, computer printer, copy machine, and telephone.

    Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.

    Must be able to transport from one site to another.
    Must be able to cover other shifts as necessary.

    Required Knowledge, Experience, or Licensure/RegistrationOne (1) to three (3) years of full-time experience, preferably in a primary care ambulatory setting.

    Bachelors degree in general management or health care management strongly preferred.
    Previous management/supervisory experience required.
    Previous experience using an electronic health record preferred.
    Proficiency in Microsoft Office.
    Bilingual Spanish/English or Polish/English, strongly preferred.
