Kaweah Health is seeking a Per Diem ASW/AMFT to provide comprehensive psychosocial assessments and treatment plans for patients. The ideal candidate will have experience working with diverse populations and strong communication skills.- Provides discharge planning, coordinating equipment and/or home care.
- Counsels individuals and their families regarding psychosocial needs.
- Makes rounds and responds to referrals from physicians and staff.
