- Lead the team in making the patient feel important and welcome in the clinic. This is done by showing the patient that we care by being constantly interested in them and by listening to the patient. It is vital that the Physical Therapist lead the team by addressing any patient concerns and handling them as they arise. The Physical Therapist is the lead on maintaining the culture here of constantly encouraging the patient and celebrating their wins.
- Evaluate the patient and find out how their condition is affecting their life. What are they unable to do and what would they like to be able to return to doing with treatment.
- Establish a plan of care with frequency and duration. A standard would be 3x a week for 6 weeks. If the condition will take longer to get better, discuss the time frame up front with the patient. For example a rotator cuff repair typically takes 3 to 4 months of outpatient PT to fully recover and return to normal activities.
- Educate the patient on the initial evaluation of their condition and how the proposed treatment will help them achieve their desired result. Discuss with the patient how pain, range of motion, and strength are all connected. Discuss how the treatment will help them with their pain, range of motion, and strength as appropriate and help them return to doing what they have been unable to do or that they have been having with difficulty or pain.
- Document fully and sign all initial evaluations, daily notes, MD progress reports, plan of care, and discharge summaries within 24 hours of date of service.
- Correct all documentation as needed to ensure that the claims are processed correctly.
- Communicate and direct the rehab technicians to ensure that the plan of care is being followed.
- Make contact with the patient at the beginning of each session to ask how they are doing and how their condition is doing. If any assessment is needed, handle the assessment and make appropriate changes to their program. Modify the plan of care as needed as patient status changes and communicate/direct the rehab technician appropriately.
- Physical Therapists will do all modifications to the plan of care and will personally monitor all new additions to the plan of care for quality.
- Physical Therapists will recognize and celebrate patient wins with the staff. They will have the patient report their win as a success story and this success story will be given to the Patient Care Coordinator to make sure that the provider receives the success story.
- At discharge the patient will be reevaluated to see if their goals have been met. If the goals have been met the patient will be discharged. If the patient has not met their goals physical therapy will be continued until goals are met.
- Outgoing, caring, positive, and high energy with an amazing ability to listen and communicate with the patient.
- You need to set a great example by being professional at all times, not critical or complaining. extremely trustworthy and approachable, but firm and disciplined.
- Result oriented, people driven, high IQ, loyal, organized, and passionate.
