    Position: Direct Hire Pharmacist

    Key Essential Functions and Accountabilities of the Job:

    • Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, lab value abnormalities, and follow-up by screening patients' charts.
    • Provide clinical consultation and clarification, resolving any concerns or questions with prescribers prior to dispensing.
    • Document all clinical activities and interventions accurately and completely, entering prescriptions into pharmacy software.
    • Verify finished prescription orders in a timely manner, ensuring accuracy while maintaining all required record-keeping and documentation.
    • Advise patients clearly and accurately regarding their medications and appropriate outcomes, verifying patient understanding.
    • Perform chart reviews for prescription refills and medication therapy management, providing drug information, drug therapy consultation, and staff education relating to drug therapy.
    • Direct pharmacy support personnel, verifying daily activities of pharmacy technicians, and serving as a role model and educator for pharmacy students and technicians.
    • Participate in clinical and administrative activities including quality control and quality improvement within the department and clinic, attending meetings and participating on committees as requested.
    • Provide health promotion and disease prevention activities related to drug use and preventative drug therapy.

    Education, Certifications, and Licenses Required:

    • Pharm.D. degree and licensure as a registered pharmacist.
    • Candidates licensed prior to 2003 with a Bachelor of Pharmacy degree will be considered with 3 years of experience in hospital and/or ambulatory clinic pharmacy. No equivalent substitution for the degree.
    • Unrestricted license as a registered pharmacist in any state.
    • Certification in Basic Life Support for Healthcare Providers (CPR and AED, may be obtained within 1 year of hire).
    • Completion of a nationally recognized (by ASHP) pharmacy residency program preferred.

    Knowledge, Skills, and Abilities:

    • Proficiency in reviewing patients' medical history, labs, and medication orders using in-depth knowledge of pharmaceutical sciences.
    • Knowledge of state and federal laws governing pharmacy practice.
    • Ability to provide care appropriate to the needs of patients of all ages.
    • Strong communication skills, both oral and written, with patients and staff.
    • Working knowledge of computer applications.
    • Math skills for accurate calculation of drug dosages and compounding formulations.
    • Ability to work cooperatively within the health system and pharmacy staff, exercise sound professional judgment, handle frequent interruptions, adapt to changes in workload and work schedule, work independently, set priorities, make critical decisions, and respond quickly to emergency requests.
