The Community Health Coordinator plays a vital role in providing comprehensive case management, care coordination, and referral services to individuals impacted by persistent mental illness, substance use disorders, homelessness, immigration challenges,
& re-entry barriers.
This role also supports HLAT's community partnerships by coordinating and reviewing Memorandums of Understanding (MOUs) with partner organizations to ensure alignment,
