About the Company:

Are you passionate about making a difference in primary care settings? We are currently seeking a Behavioral Health Therapist to join our team at Ithaca Family Medicine, Alma Internal Medicine, and Shepherd Family Medicine. As a core member of our collaborative team, you will play a vital role in supporting the mental and physical health care of our patients.

Responsibilities:

• Assess clinical status of patients and intervene to mitigate further deterioration
• Provide brief behavioral interventions using evidence-based techniques
• Assist in program development and patient education
• Track patient follow-up and outcomes using a registry

Other Duties and Responsibilities:

• Eliminate barriers to treatment and improve resource utilization
• Facilitate referrals for necessary services outside the health system
• Conduct family meetings and integrate relevant theories into casework
• Collaborate with specialists and ensure compliance with regulatory requirements

Certifications and Licensures:

MSW - Masters of Social Work

Preferred Equivalent Experience - Licensure as a Licensed Psychologist or Masters Social Worker in the State of Michigan.

PCSW - Psychologist/Counselor/Social Worker

Required Equivalent Experience - Licensure as a Licensed Psychologist or Masters Social Worker in the State of Michigan.

Required Education:

Education: Masters Degree

Comments: Knowledge of psychology or social work acquired through completion of a MA or Ph.D./ psychology or master's degree in social work.

Experience, Training, and Skills:

• Membership in at least one professional organization preferred
• Training and experience in casework principles and therapy techniques
• Knowledge of Integrated Behavioral Health practices
• Interpersonal skills and adherence to professional codes of ethics

Physical/Mental Requirements and Typical Working Conditions:

Exposure to stressful situations and ability to handle multiple tasks

Able to wear personal protective equipment

Must follow required health testing and demonstrate competency in relevant technologies

Overall vision and hearing necessary with or without assisted devices

May require frequent postural changes and some exposure to infectious material

Ability to work independently and maintain regular attendance

All other duties as assigned.
