    Company Description: The mission of our Residential Services Department is to provide housing and living services to the disadvantaged and developmentally disabled. Our Residential Homes or Intermediate Care Facilities can house up to 15 individuals and CILA's or Community Integrated Living Facilities can house between 6-8 residents. Both options offer safe, small group environments where residents can learn the necessary life skills such as money management and personal care under the guidance of trained 24-hour staff.

    Summary: Under the direction of the Health Services Supervisor, the Health and Services Coordinator assumes primary responsibility for the medical appointments for ECSS Residential individuals.

    Principal Duties/Responsibilities:

    • Ensure delivery and/or coordination of all Residential services are in compliance with DHS Rules 115 & 116, CILA & ICF, agency mission, agency policy & procedure, program guidelines, and best practice
    • Completes documentation as required by DHS rules and regulations and Ada S. McKinley policies and procedures.
    • Schedule, accompany/drive participants to medical appointments for all Ada residential programs approximately 3-4 days a week as directed.
    • Assists with scheduling and transportation for medical/dental appointments ensuring follow through with recommendations and reporting effects and side effects of medications.

    Requirements:

    • This position requires Associate's degree (A.A.); or a combination of comparable education and experience that provides the necessary knowledge, skills, and abilities to perform the functions of the position proficiently.
    • Minimum of two years of supervisory experience and cumulative experience with increasing responsibility within a human services or social services working with individuals with development disabilities.
    • This position requires the incumbent to be on-call after normal business hours to address any emergencies and/or concerns.
    • This position must possess valid/current driver's license and annual clearance of the Agency background screening process.

    The incumbent must be able to effectively communicate orally and in written with internal/external vendors/clients, staff and management, and maintain professional relationships.

    This position requires the individual possess the ability to partner with clients individually and/or in family to help them mobilize resources and build resilience.

    This position requires the individual possess the ability to coordinate effectively with other team members, other Agency resources and outside services in addressing client needs.

    This position requires the individual possess an understanding of evidence-informed practices, engagement strategies, boundary setting, concurrent documentation, working in a community setting, organizing, motivational interviewing, and family systems.

    Mental/Physical Demands: Ability to conceptualize and solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. While performing the duties of this job, the employee is regularly required to use hands to finger, handle or feel and talk or hear. The employee frequently is required to sit. The employee is occasionally required to stand and walk. The employee must lift and/or move up to 10 pounds and occasionally life and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and ability to adjust focus.

    Environmental Conditions: The noise level in the work environment is usually moderate. Duties are performed in a typical office environment. No unusual or adverse working conditions exist.
