    Description


    The Intensive Program Clinician I, provides direct intensive community treatment services for individuals in the REACH program diagnosed with behavioral health diagnoses. Works as part of the REACH team to ensure services and resources are coordinated among providers as needed to help individuals served live and participate optimally in the community.

    Clinician I-Starting Pay: $50,956-Depending on Salary

    Clinician II-Starting Pay: $64,035-Depending on Salary

    Responsibility
    • Works with team members to write and update required assessments of needs and strengths to meet the admission and ongoing provision requirements for REACH program. Ensures the proper pre-authorization documentation to qualify for and receive payment for all services rendered are submitted within allowable time frames and maintained in the record.
    • Writes effective and measurable Individualized Service Plans (ISPs) as required upon admission, as the needs of individuals served change, and at least once annually thereafter. Works to ensure supports and interventions are provided as stated in the plan. Ensures all assigned plans are reviewed at least quarterly.
    • Provides counseling within scope of practice. If qualified and trained, completes psychological testing. Provides for and assists with coordination of treatment and services. Provides for continuity of care including discharge, referral, placement, follow-up appointments, and liaison services.
    • Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans. Advocates for services on behalf of individuals served and their families/care givers as needed. Documents and reports suspected violations of Human Rights according to NRVCS policies.
    • If assigned to REACH Crisis Therapeutic Home (CTH), correctly provides medication administration services to individuals supported as prescribed and in accordance with all medication regulations. Provides assistance with other medical procedures (i.e.: blood sugar levels, wound care, etc.) as ordered by a physician and approved by supervisor. Completes a Medication Administration Incident Report Form by the end of shift for any problems that involve medications. (Note: Part Time/Relief staff will be trained to administer medication based on program needs. Staff not formally trained and certified by NRVCS may not administer any form of prescribed or over the counter medications.)
    • Maintains complete and accurate medical records, and completes all administrative documentation according to policy and procedures. Maintains all communications, both written and verbal, with respect and confidentiality and complies with HIPAA standards and regulation.
    • Collaborates with other agencies and NRVCS programs to ensure that all services reflect the individuals' needs. Monitors satisfaction with services provided and helps individuals served complete satisfaction surveys as needed.
    • Promotes independence by teaching and modeling life skills such as: personal hygiene, nutrition, understanding of mental and physical health and well-being, methods to access assistance, financial management, coping skills, self-esteem and self-reliance.
    • Ensures all safety, health, and universal precautions are properly addressed and monitored. Completes NRVCS Incident Report forms by the end of the shift for any observed or suspected incidents that involve individuals served or staff welfare and/or safety.
    • Provides psycho-education to individuals served and families/care givers to promote better understanding of behavioral health related topics. Works to establish linkages between individuals served and naturally occurring supports in their family and community. Participates in on-call rotation.
    • Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Conducts self in a professional manner at all times. Participates in team meetings unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail. Serves on designated teams, committees and work groups.
    • Performs at the expected levels of individual and team productivity. Ensures all client records are in compliance with applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid.
    • Ensures all required certifications and/or licensure are kept up to date. Attends/provides other training as required to meet personal professional development and/or program needs.
    • Strives to promote continuous quality improvement of services through the timely completion of QA activities (i.e. Level II reviews, satisfaction surveys, outcome measurement reports, etc.). Works with team to ensure all services are provided at the expected levels of quality and are in compliance with all applicable agency policies as well as any local, state, and federal requirements including guidelines as they pertain to Medicaid reimbursement.
    • Completes other related tasks as assigned by Supervisor, Program Manager, Program Director and/or Senior Director.
    Qualifications
    • Extensive knowledge and training in best practices for providing community-based crisis services for individuals with co-occurring developmental disabilities and behavioral health diagnoses residing in community settings.
    • Demonstrated knowledge of agency, local, state, and federal regulations as they pertain to the REACH program; Medicaid guidelines; standards for clinical services; and related human rights, ethical, and legal concerns.
    • Thorough knowledge of DSM-V etiology, symptoms, treatment, and prognoses.
    • Knowledge of and ability to administer various treatments and therapeutic modalities as appropriate and/or prescribed.
    • Ability to communicate effectively orally and in writing and to prepare and present educational materials.
    • Basic working knowledge of pertinent medical, legal, and financial terminology; and psychopharmacology.
    • Demonstrated ability to work independently and as a team; think critically and problem solve effectively; and recognize and respond to crises appropriately.
    • Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment.
    • Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment;
    • Must have acceptable drug screen results upon hire.
    Education/Experience Required:
    • Graduation from an accredited university with a Master's Degree in social work, psychology or human services related field is required.
    • State of Virginia Professional licensure eligibility required.
    • At least one year of clinical experience working with individuals with serious mental illness, substance use disorders, developmental and/or intellectual disabilities, preferred.
