Role Description

This is a full-time hybrid Care Coordinator role with Soran Health located in Miami, FL, with flexibility for remote work.

As a Care Coordinator, you will work with healthcare professionals to manage care for patients, perform health assessments, and navigate patients to the proper resources.

You will also need to develop and implement individual care plans and schedule appointments for patients.

Qualifications
Prior work experience in patient care coordination or related fields
Strong communication skills, both written and verbal, and adept at collaborating with healthcare teams
Proficient in Microsoft Office Suite and various electronic health records (EHR) platforms
Ability to multitask and prioritize tasks based on patient needs and organizational goals
Understanding of healthcare regulatory requirements, policies, and procedures
Bilingual ability highly preferred
