Job summary
Provides skilled Occupational Therapy service on an intermittent basis to clients in their homes under the supervision of the Occupational Therapist.
Responsibilities
- Assists in evaluation and development of the rehabilitative plan of care and in periodic re-evaluations as required under the direction of the OT.
- Participates in preparation of clinical daily progress notes which are reviewed by OT enters EMR within 24 hours also prepares summary reports assists OT providing instructions client family demonstrating exercise programs guides instructs prescribed therapeutic self-care methods devices creative activities directed toward improving independence physical mental functioning identifies reports possible ineffective drug therapies adverse reactions significant side effects drug allergies contra-indicated medications to OT observes records activities findings clinical record reports clients reactions treatment changes condition to OT physician consults regarding outcome home visits writes physical findings instruct other health team personnel including home health aides family members appropriate phases occupational therapy so they may work with client attends rehabilitation department staff meetings case conferences instructs client care use wheelchairs braces splints prosthetic orthotic devices participates discharge activities assists completing Occupational Therapy Discharge Summary confirms weekly scheduling visits Supervisor Director Nursing coordinate Agency visits assumes responsibility self-development continually striving improve healthcare knowledge through educational programs attendance workshops conferences active participation professional organizations individual research reading assumes duties deemed necessary suitable by OT maintains efficient flow Agency work reporting assigned scheduled items follows procedures notification absence time off requests maintains current TB testing refer Health Screening Policy Seniority level Entry level Employment type Part-time Job function Health Care Provider Industries Hospitals Health Care
