    Family Circle of Care is an FQHC certified, non-profit, 501 (c)(3) health care center with clinics located in Tyler, Jacksonville, and Athens, Texas. Through our integrated care model we demonstrate significant improvements in health outcomes for patients and valuable cost savings to the health care industry, including Medicaid programs, as well as through the provision of care to the uninsured.

    We have an excellent opportunity for a proactive go-getter who thrives being part of a team. We are seeking a Family Medicine Nurse Practitioner for our Tyler location.

    A summary of your activities would include:
    • Perform history and physical exams
    • Formulate working diagnosis and appropriate treatment
    • Prescribe medications as permitted by TX law
    PROFESSIONAL REQUIREMENTS
    • Degree from an accredited Nurse Practitioner Program
    • A current, unrestricted license from the Texas Board of Nursing to practice as a Nurse Practitioner in the State of Texas
    • Registered and have current DEA, DPS and other certificates to legally operate as a Nurse Practitioner in the State of Texas
    • Current CPR for Healthcare Professional Certification also required
    POSITION REQUIREMENTS
    • Maintain medical expertise and competency (to include age specific competency)
    • Demonstrate through knowledge of procedures to deliver quality primary medical care
    • Follow established protocols and exercise professional judgment
    • Excellent Customer Service and Communication skills, bilingual in Spanish preferred
    • One year of practice experience providing direct patient care, preferred
