What you will be doing
As a dedicated social worker, you will serve as a vital liaison among residents, families, staff, and outside agencies—ensuring compassionate communication while handling sensitive and confidential information with professionalism. This social worker role includes completing admissions, conducting social service evaluations, coordinating referrals, and providing essential support with financial and legal matters such as pension applications, insurance communication, and Medicare/Medicaid needs. You will perform key assessments including BIMS, PHQ-9, behavioral health screenings, and substance use evaluations, while also maintaining accurate, timely documentation in MDS care plans and leading care conferences to support personalized resident care.
In this rewarding social worker position, you will guide residents and families through discharge planning, connecting them with home care services, community resources, and safe transition options. You will also provide or coordinate counseling services, psychiatric medication support, and ensure resident satisfaction by monitoring the Grievance and We Care Connect programs. The role offers independence and variety—allowing you to plan and prioritize diverse projects, collaborate across departments, and contribute to a warm, responsive environment through excellent customer service. About you
- Bachelor's degree, preferred
- Must be at least 18 years of age.
3-5 years of social services work experience in long term care, preferred
Proficiency in word processing, telephone operation, typing, and office equipment.
Familiarity with computer software, including spreadsheets, databases, and presentation software.
