    We are searching for a Case Manager for the North Bay area.

    General information of position:
    • Providing supervision of Direct Care staff and coverage coordination.
    • Participate in the hiring, training and supervision of staff.
    • Attentiveness of reportable events and submitting reports within required time frames.
    • Manages and monitor client finances within required time frames.
    • Serve as liaison with service coordinators, families, other agencies, and internal staff.
    • Manages client caseloads, weekly review of client charts/records, and monitoring Daily Shift Notes.
    • Manages the supervision and evaluation of service delivery quality and content program to clients.
    • Coordinates and ensures staff and the program's daily activities are completed efficiently.
    • Attends all County/community meetings, as assigned, provides leadership, and communicates all information with appropriate staff in a timely manner.
    • Must maintain current working knowledge of Supported Living Services Federal & State laws, including familiarity with Title 17 regulations.
    • Maintains a prominent level of confidentiality.

    Qualifications:

    Education: Degree is great but not required. We are looking for experience.

    Experience: People Management experience

    Experience in case management working for companies providing services for disability individuals.

    Experience working with Mental Health/Intellectual Disabilities or

    This has the potential for growth in a solid career field.
