    Travel Registered Nurse (RN) – Case Manager

    Apple Valley, CA

    Duration: 13 Weeks

    Shift: Days – 5x8s (8:00 AM – 4:30 PM)
    Weekend Rotation Required (every other weekend)

    Important Requirements

    • Seasoned traveler required (HARDSTOP – no first-time travelers)
    • Must have 2+ years Acute Care Case Management / Discharge Planning
    • Behavioral health–only experience does NOT qualify
    • Active CA RN license (pending accepted but must be active by start date)
    • Epic experience required

    Position Overview

    Seeking an experienced Acute Care RN Case Manager to manage a caseload of up to 25 patients in a hospital setting. This role focuses on care coordination, utilization review, and complex discharge planning across diverse patient populations.

    Core Responsibilities

    • Patient assessment & needs evaluation
    • Care coordination & discharge planning
    • Utilization review & admission criteria
    • Participate in treatment plan development
    • Organize & lead family meetings
    • Arrange DME, SNF placement & ambulance transport independently
    • Prior authorizations & benefits eligibility review
    • Disease management & plan of care oversight
    • Regulatory compliance (CMS, DRGs, Joint Commission standards)
    • Document in Epic EMR

    Required Experience

    Acute hospital case management experience (ED, ICU, MS, Surgical Services)
    Utilization Review
    Knowledge of HMOs, IPAs
    Familiarity with:

    • Adoption/foster care processes
    • Substance abuse treatment resources
    • Homeless patient discharge coordination
    • Mental health crisis support
    • Advance directives & POLST
    • Protective services processes
