- Responsible for collaboration, communication & facilitation of optimal provision of care with assistance of computerized &/or digital platform that supports the daily management of care.
- Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.
- Utilize clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care.

- Graduation from an accredited school of nursing
- Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing preferred, according to NYS requirements.

- Maintains all licensures and certifications according to NYS regulations and specialty area requirements.

Details:
Come join our amazing multidisciplinary team on Unit 3-1, an acute inpatient/GI-GU surgical unit.
Our nurses work closely with the hospitalist physicians and Wound Ostomy Nurses (CWONs) to provide safe, competent, and compassionate care for a diversified patient population.
This incorporates the assessment, planning, intervention, and evaluation of individualized care plans for patients with a variety of medical illnesses and surgical procedures.
Responsibilities and specialized training include:
direct patient care, cardiac monitoring, arrhythmia interpretation, specialized wound care, peritoneal dialysis, ostomy care and care of wound vacs.
