- Coordinate the process of completing state-required assessments that determine payments from Medicare, Medicaid, and Managed Care by performing ongoing compliance related to Skilled Nursing Services.
- Maintaining accurate and timely documentation to comply with state and federal regulations.
- Collaborating with the interdisciplinary team to develop and implement individualized care plans.
- Monitoring resident progress and adjusting care plans as needed.
- Providing education and support to staff on assessment processes and regulations.
- A valid Registered Nurse (RN) license in Pennsylvania.
- At least 2 years of experience in long-term care nursing.
- Excellent communication and interpersonal skills.
- Ability to use the computer system to enter assessments and create care plans.
- A commitment to quality care and resident-centered practices.
