    Title: RNAC (Registered Nurse Assessment Coordinator)

    Status: Full Time

    Shift: 8:00 am - 4:30 pm, Monday - Friday

    In this role, you will ensure the quality of care for our residents by:

    • Coordinate the process of completing state-required assessments that determine payments from Medicare, Medicaid, and Managed Care by performing ongoing compliance related to Skilled Nursing Services.
    • Maintaining accurate and timely documentation to comply with state and federal regulations.
    • Collaborating with the interdisciplinary team to develop and implement individualized care plans.
    • Monitoring resident progress and adjusting care plans as needed.
    • Providing education and support to staff on assessment processes and regulations.

    To be successful, you will need:

    • A valid Registered Nurse (RN) license in Pennsylvania.
    • At least 2 years of experience in long-term care nursing.
    • Excellent communication and interpersonal skills.
    • Ability to use the computer system to enter assessments and create care plans.
    • A commitment to quality care and resident-centered practices.
