- Work with the interdisciplinary team to execute a customized plan of care for each unique patient.
- Guide the patient through the treatment process, reviewing instructions, and scheduling appointments.
- Facilitate interaction between patients and their physician and referring physician(s) as appropriate.
- Navigate the patient and family across care settings according to the established patient care plan and clinical pathways
- Utilize effective communication to ensure patient and caregiver(s) have appropriate information, understand, and have the capacity and tools to adhere to the individualized treatment plan.
- Contribute to quality-driven performance improvement initiatives.
- Bachelor's Degree Nursing
- Current New York State Registered Professional Nursing License
- Excellent communication (both oral and written), interpersonal, and organizational skills.
- Demonstrate consistent professional conduct and meticulous attention to detail.
- Proficiency in Electronic Health Record documentation
- Minimum of two years experience in an outpatient medical office setting.
- Healthcare or Customer Service Experience
- Patient Navigation Certification (NYCBS will fully support our team member(s) in achieving and maintaining specialized certification.)

In this role you will:
