    As an In-Community Support Professional, you will provides in-home and community supports to individuals of all ages with developmental/intellectual disabilities and their families located throughout Southern New Jersey.

    The program helps families caring for a loved one challenged with communication, social interaction and behavior difficulties. In-home services give families the opportunity to learn together in a safe, supportive environment.

    The ultimate goal is to improve the quality of life for the entire family and allow individuals to reach their full potential while living in the community.


    Schedule:
    Per diem shifts available


    Shifts:
    morning, afternoon, evening, and/or weekends. Hours are flexible and will vary based on caseload


    Responsibilities:

    • Provide direct one on one care, supervision, and assistance to individuals using Positive Behavior Supports
    • Support and assist individual in accomplishing daily living skills, positive behavior, socialization, and other identified needs
    • Accompany individual into the community
    • Crisis prevention, recognition, and intervention
    • Staff meeting attendance
    • Ensure all agency policies implemented and followed
    • Complete required trainings
    • Complete progress notes within 24 hours of service provision

    Qualifications:

    • High School diploma plus 6 months experience working with adults with developmental or intellectual delays; Ability to work days, afternoons, evenings, and/or weekends
    • Valid Driver's License required
    All positions must have a valid driver's license and must be 18 years old or older

    All positions require a valid driver's license in good standing and pre-employment drug screening.

    All candidates, if hired, will be expected to cooperate with Oaks Integrated Care and The Department of Human Services in all inspections and investigations.
