    Description

    Overview:
    The
    LPN/MA Care Coordinator is responsible for patient care within the setting of the clinic environment.

    The Care Coordinator position also assist patient with managing their health care needs remotely (via telephone, or video calls).

    They are trained and capable of performing patient care for the age groups newborn to 105 years old.

    The
    LPN/MA Care Coordinator reports directly to the RN Director of Clinic Nursing.


    Responsibilities:

    • Take vital signs; obtain medical history, gives injections and immunizations as ordered by the clinic providers.
    • Maintain required logs and information needed for quality control.
    • Performs Medicare Annual Wellness (AWV); implementation and oversight of a Medicare Chronic Care Management and Transitional Care Management Programs.
    • Work quality measures for improvement of patient care measures for MIPS/MACRA, HEDIS, individual payer source reports, registries in Meditech, and other resources to improve quality measures for the Medical Group, provider and patients.
    • Collaborates with clinic providers and staff to strengthen data quality, collection, analysis, and reporting of health information.
    • Other duties as assigned

    Qualifications:

    LPN
    Education
    Graduate of an approved Practical School of Nursing.
    Licensure/Certification
    Current licensure by Missouri State Board of Nursing.
    BLS certification required within 6 months of employment
    Work Experience
    Successful completion of orientation for
    LPN.

    MA

    Education:
    High School graduate.
    Prefer graduate of a Medical Assistant program, but not required.

    Licensure/Certification:
    Certification as a medical assistant preferred.
    Basic Life Support (BLS) required

    Work Experience:
    Minimum 1 year working as a medical assistant in a practice. Graduation from a medical assistant program may be substituted in lieu of the 1 year work experience.
