    Job DescriptionSalary: $19 - $24

    POSITION: Bi-Lingual CMA or LPN

    The CMA or LPN is responsible for supporting all aspects of patient care offered by Bradley Free Clinic, specifically intake and triage, chart review and project management, nurses' station and discharge. The CMA or LPN must work as a team member to ensure safe, effective and efficient operations. The CMA or LPN will also provide direct clinical support to staff and volunteer providers during day and/or evening clinic, track patient data, and facilitate outside referrals for continued care.

    DUTIES

    Intake/Welcome

    • Administer full intake/triage, including vitals, medical history, patient preferences, etc. and enter into Athena;
    • Work with volunteers, interns, externs, etc. to ensure they are properly trained on intake process, and delegate duties to volunteers/students as possible; and
    • Room patients and communicate with providers regarding patient arrival and departure.

    Office--Chart Review and Projects

    • Conduct chart review on providers on schedule and as assigned;
    • Ensure outstanding orders are linked to results by either processing documents, or adding internal notes and closing outstanding orders; and
    • Manage assigned project such as women's health, diabetes management, colonoscopy screening and Hep-C, etc, ensuring you are keeping track of patient phone calls, tracking and Athena updates, and engaging volunteers in project as possible.

    Nurses' Station/Patient Navigator

    • Answer and respond to phone calls and messages;
    • Ensure all messages are retrieved and responded to before end of shift;
    • Respond to provider needs during clinic including running stat labs, EKG, injections, vaccines, procedure prep, etc.;
    • Clean and stock rooms; and
    • Navigate patients to lab, pharmacy, etc. as needed.

    Discharge

    • Meet with each patient before they leave clinic, reviewing chart notes and orders to ensure all provider instructions are followed;
    • Confirm with patient they have completed all necessary steps (ie lab, vaccines, pharmacy pick up);
    • Follow up on any outstanding notes in chart (ie FIT follow up, vaccine recommendations, etc) to ensure patient is aware and has completed if possible;
    • Call to schedule imaging or outside referrals while patient is in clinic if possible;
    • Print orders as needed for patient to follow up (ie XR or outside lab orders);
    • Confirm all follow ups are entered into chart;
    • Communicate next steps and expectations, and ensure patient goes to check out.

    Other duties as assigned by clinical manager

    • Attends scheduled meetings, including monthly medical team meeting;
    • Stock rooms and manage supplies, including donated and purchased supplies;
    • Call patients with lab results or send letters as needed;
    • Perform STAT labs, COVID testing, and administer vaccines;
    • Send outside referrals and follow up with patients and outside providers;
    • Schedule imaging appts;
    • Scan, upload, and appropriately label documents; and
    • Handle tasks placed in your clinical inbox.

    QUALIFICATIONS

    • Minimum of 1 year of providing patient care in an in-or out- patient medical facility as a CMA or LPN with Associate's degree, CMA or CCMA; and
    • Experience working on a team and ability to work independently;
    • Excellent communication skills with patients, staff, and volunteer health care practitioners;
    • Patience, understanding, and compassion in direct interactions with patients; and
    • Highly organized and detail oriented;
    • Must have to ability to maintain confidentiality at all times;
    • Must be self-motivated with good organizational skills;
    • Ability to maintain composure when faced with difficult situations;
    • Must be able to handle confidential situations with tact;
    • Ability to work with computers, electronic healthcare records, email and calendar programs, other programs including Excel and Google Drive, etc.

    BENEFITS

    Medical Insurance: Participating employees contribute 15% of the employee medical insurance premium while Bradley Free contributes the remaining 85%.Employees are responsible for 100% of dependents' medical insurance premium. Employees who work 30 hours on a regular basis will be eligible for medical the first of the month following 60 days.


    Dental Insurance: Participating employees contribute 15% of the dental insurance premium while Bradley Free contributes the remaining 85%. Employees are responsible for 100% of dependents' dental insurance premium. Employees who work 30 hours on a regular basis will be eligible for dental the first of the month following 60 days.


    Vision Insurance: Participating employees are responsible for 100% of the insurance premium. Employees who work 30 hours on a regular basis will be eligible for vision the first of the month following 60 days

    401K: Bradley Free Clinic will match employees' contributions 100% up to 3% of the employee's annual salary and 50% on the next 2% of the employee's annual salary. Employees are eligible to participate once they attain age 21 and have 1 year service with the company. You will enter the plan on the first day of the month on or after you meet the eligibility requirements. Our 401(k) plan has a three-year cliff vesting schedule. This means you are always 100% vested in contributions you choose to defer, however, you must remain employed for three (3) years to be 100% vested in any employer matching contributions; there is no step vesting schedule.
