    Description The Service Coordinator assists families with a child aged birth to 3 years of age with a disability in accessing a community-based, comprehensive system of services.

    These services are individually tailored to meet the families' needs and are provided in the child's natural environment, such as the family home, a learning setting or similar setting.

    The Service Coordinator coordinates ongoing eligibility and the Individualized Family Service Plan (IFSP), serves as the family advocate, and is the initial resource for Early Intervention Services under Part C of IDEA.The Service Coordinator will conduct in-home assessments, lead the IFSP and teams which include, but are not limited to, the family, medical providers, Eligibility Determination Team, educational providers, and other community-based resources, through the process of problem-solving, negotiation, and resolution of conflicts to provide the best options available for the child and family.

    Position Requirements This position requires a minimum of a Bachelors's Degree from an accredited university or program, preferably in a human services-related field such as Social Work, Psychology, Education, or Nursing.

    Related majors will be considered, especially with personal or work experience related to social services.

    Travel is required so the employee must have reliable transportation, a valid driver's license, adequate insurance, and a good driving record.

    Mileage is reimbursed.

    The Service Coordinator must meet and maintain state credentials as a Service Coordinator Specialist under First Steps regulations and remain in good standing with the State of Indiana.

    The employee must demonstrate competence in the resource and referral process (such as DCS, Head Start, Healthy Families, CSHCS, and Medicaid) to assist families adequately and fully.

    The successful individual has good communication skills, both verbal and written to interview families for eligibility and ongoing services, adequate computer skills to complete mandatory documentation, above-average time management skills, and the ability to work independently with little supervision.

    The Service Coordinator works 40 hours per work and as long as the employee meets required deadlines, may be allowed to make his/her own schedule.

    Aging and In-Home Services - as the local Area Agency on Aging, Area 11, Thrive Alliance provides Care management and resources to assist those over the age of 60 to remain at home or in a community setting.

    Aging and Disability Resource Center (ADRC): a highly visible and trusted place where people can turn for information on a full range of support options
