    Hospice Clinical Manager

    We are seeking a Hospice Clinical Manager (RN) for a full-time opportunity with our team in San Marcos, California

    The Hospice Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Associate Director of Patient Care Services is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.

    Compensation Package:

    • $120,000 to $135,000 Per Year (Based on Experience)

    Requirements:

    • Registered Nurse Licensed in the State of California
    • 5+ Years of RN Leadership Experience - Ideally in the Hospice Setting
    • Minimum Associate's Degree in Nursing
    • Utilizing Kinnser - Training Offered
    • Spanish Speaking Individual Preferred but NOT Required
    • Maintains a current CPR and BLS certification
    • Current and Active Driver's License
    • Must Have a Compassionate and Can-Do Attitude

    Responsibilities:

    • Monday through Friday 8 AM to 5 PM
    • Tuesday Mandatory In-Person Day
    • Attending Rotating Tuesday/Thursday In-Person IDG Meetings
    • Remaining Week Days in Field or Working Remotely
    • Provide Home Hospice Visits When Needed To Educate Other Nurses
    • Minimal Call By Phone Only for Emergencies
    • Manage a Team of Nurses, LVN's and HHA's
    • Receives Case Referrals
    • Reviews available patient information related to case, including home visits, to determine hospice/home care needs.
    • Assigns appropriate hospice personnel to case as needed. Conferences with Medical Director regarding any questions about an individual's eligibility for services.
    • Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians
