- Performs thorough interviews with patients and family members with appropriate professional language.
- Conducts telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history with appropriate telephone etiquette.
- Interprets and enters "Prior to Admission" medication history into the electronic medical record in an accurate and timely manner.
- Coordinates responses and resolutions to issues with appropriate internal and external parties.
- Regularly and appropriately communicates with supervising pharmacists for any questions and additional work assignments.
- Accurately tracks interventions and productivity using department's process.
- Maintains proficiency with departmental information system technology and documentation systems.
- Maintains contemporary professional knowledge base.
- May represents department on various hospital committees.
- Participates in evaluation of quality service, development, and implementation of improvement.
- Communicates with other hospital employees and departments as necessary.
- High School Diploma or GED (Required)
- One of the following is required CertIfications/Licensures: (Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA), Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Intern (IPLA) you do not need to be nationally certified with PTCB or NHA.)
