- Assist in maintaining a safe environment
- Assist in activities of daily living, such as feeding, bathing, dressing, and ambulating, as outlined in the personalized care plan
- Promotes the patient's mental alertness through involvement in activities of interest
- Report any changes in the patient's mental or physical condition or home situation to the Case Manager and members of the interdisciplinary team promptly
- Teaches basic personal care to the primary caregiver
- May provide care in the patient's home environment or long-term care or assisted living facility
- Completes at least 12 hours of paid in-service training per calendar year
- Practices basic infection control measures
- Must hold a Certified Home Health Aide license in the state of New Jersey.
- Must be able to lift at least 25 lbs.
- Must have a reliable car, as this is a driving position. May be asked to drive outside of your regular route to cover call-outs or other team members' paid time off.
- Must have a warm, compassionate, patient demeanor.
