Home Health Aide (HHA)

Home Health Aides only provide care outlined in the personalized care plan.

Responsibilities:

• Assist in maintaining a safe environment
• Assist in activities of daily living, such as feeding, bathing, dressing, and ambulating, as outlined in the personalized care plan
• Promotes the patient's mental alertness through involvement in activities of interest
• Report any changes in the patient's mental or physical condition or home situation to the Case Manager and members of the interdisciplinary team promptly
• Teaches basic personal care to the primary caregiver
• May provide care in the patient's home environment or long-term care or assisted living facility
• Completes at least 12 hours of paid in-service training per calendar year
• Practices basic infection control measures

Qualifications:

• Must hold a Certified Home Health Aide license in the state of New Jersey.
• Must be able to lift at least 25 lbs.
• Must have a reliable car, as this is a driving position. May be asked to drive outside of your regular route to cover call-outs or other team members' paid time off.
• Must have a warm, compassionate, patient demeanor.
