- Facilitate new membership process, including responding to initial inquiries, conducting intakes and assessments, supporting visits, assessing financial need, etc.
- Ensure timely and thorough documentation and communication with clients, their support system, and colleagues
- Support members and caregivers in accessing community resources
- Collaborate with Program Director to continuously assess and improve processes and tools
- Assist in the development and implementation of social work/human service internship projects and experiences to enrich member services
- Contribute to ongoing program evaluation and research efforts
- Produce reports
- Perform additional tasks and projects as required
- 3+ years client-facing services required; community mental health and resource referral experience strongly preferred
- Experience providing client services in an online environment
- Professional experience working with people living with acquired brain injury, supporting people with diverse mental health needs, as part of a multi-disciplinary team in a clinical setting, and/or student fieldwork supervision strongly preferred
- Compassionate self-starter with proven communication skills
- Top-notch organizational and time management skills
- Readiness to support individuals experiencing trauma or crisis
- Ability to triage and prioritize projects and tasks in a fast-paced environment
- Microsoft Office 365 proficiency, client database experience
