    Carolina Health Centers, Inc. is looking for a Mental Health Counselor (MHC) to grow their Behavioral Health Department and join their pediatric primary care team.

    Carolina Health Centers, Inc. is a decorated patient-centered medical home that serves children and families throughout the Lakelands area. The MHC will provide ongoing clinical counseling as an integral member of a multidisciplinary care team at The Children's Center in Greenwood, South Carolina.

    The MHC will

    • Conduct screenings and assessments with our patient population
    • Provide skilled ongoing therapy for pediatric patients (ages 5-21)
    • Assist the primary care provider in recognizing, treating, and managing behavioral health issues
    • Utilize evidence-based interventions
    • Clarify mental health diagnoses, when needed
    • Collaborate with other medical professionals in a fast-paced work environment
    • Connect patients with additional resources, as needed
    • Provide same-day warm handoffs as well as real-time consultation, when possible
    Requirements:

    Minimum Qualifications

    • Master's degree and full professional license in the state of South Carolina: LMSW, LISW-CP, LPC, LPCS, LMFT, LPC-A, LMFT-A

    Duties & Responsibilities

    • Utilize clinical skills to accurately assess and formulate diagnosis as well as treatment recommendations for patients
    • Develop specific treatment plan to guide patient's services
    • Provide individual or family therapy, as clinically indicated
    • Respond to the mental health needs of children, youth, and adults
    • Communicate effectively with an interdisciplinary care team
    • Provide treatment utilizing evidenced-based modalities (CBT, DBT, Motivational Interviewing, etc.)
    • Communicate patient updates in a clear, concise manner
    • Collaborate with referring medical provider to support patient's overall health goals
    • Coordinate patient care with community partners
    • Document patient encounters in a timely and efficient manner
    • Participate in department meetings, as scheduled
    • Complete administrative tasks in a timely manner

    Salary:

    Starting at $56,911.32 annually
