Job summary
We are seeking a Community Support Specialist to assist consumers with monitoring mental health symptoms and provide services necessary to enable them to live work and recreate in the community environment.Responsibilities
- Assess the needs of clients and develop goals that will allow them to achieve the greatest level of independence in their community.
- Work with clients and other team members to creatively explore alternatives to hospitalization higher level of care homelessness and incarceration.
