    Summary

    We are hiring for a Registered Nurse (RN) Case Manager in Hospice, working Full Time. At Casa de la Paz Hospice in Sierra Vista, we value a culture of caring and meaningful connections with patients, families, colleagues, and the community.

    Responsibilities

    The RN Case Manager assumes full nursing responsibility for delivering the Plan of Care for each hospice patient, continuously evaluating personal and professional performance, and making necessary changes to increase productivity and quality of care.

    • Makes the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
    • Identifies and re-assesses patients' physical, psychosocial, and environmental needs every 15 days.
    • Initiates and coordinates the plan of care, maintaining accurate clinical notes regarding the patient's condition.
    • Documents problems, goals, interventions, and patient/family response to hospice care.
    • Collaborates with patients, families, attending physicians, and other members of the IDG to provide daily patient and family care.

    Education and Experience

    Experience Requirements

    • One year of clinical experience.

    License Requirements

    • Current RN licensure in the state of practice and one year of clinical experience.
    • Current CPR Certification.
    • Valid driver's license, vehicle insurance, and access to reliable transportation or public transportation.
