Licensed Vocational Nurse (LVN) – Outpatient Direct Primary CareAbout Harbor Direct Primary Care

At Harbor Direct Primary Care, we provide personalized, relationship-driven primary care in a modern outpatient setting. Our model prioritizes accessibility, prevention, continuity, and meaningful patient engagement. We are seeking a compassionate, organized, and proactive Licensed Vocational Nurse (LVN) to join our growing team and help us deliver exceptional, patient-centered care.

This is an ideal opportunity for an LVN who enjoys building long-term relationships with patients, working closely with a small collaborative team, and practicing in a lower-volume, high-quality care environment.

Position Overview

The LVN will play a central role in supporting clinical operations, coordinating care, and enhancing the patient experience. This position focuses on outpatient primary care services including preventive visits, chronic disease management, minor procedures, and care coordination.

Key Responsibilities

Conduct intake assessments including vital signs, medication reconciliation, health histories, and preventive screening updates.
Assist the MD with in-office procedures such as:
Minor wound care and suturing support
Injections and immunizations
Phlebotomy and specimen processing
EKGs and point-of-care testing
Administer medications and treatments within LVN scope of practice..
Support chronic care management for conditions such as hypertension, diabetes, hyperlipidemia, and heart disease.
Provide patient education regarding medications, lifestyle modifications, and preventive care plans.
Coordinate referrals, diagnostic testing, and follow-up care.
Manage inbox messages, lab results, and patient communication through the EMR system.
Maintain accurate and timely documentation in the electronic health record.
Ensure compliance with HIPAA regulations and clinic privacy standards.
Assist in maintaining clinic workflow, supply management, and quality improvement initiatives.

Qualifications

Active and unrestricted LVN license in good standing.
Outpatient primary care or family medicine experience preferred.
Strong phlebotomy and injection skills.
Comfortable working in a fast-paced yet patient-focused clinical setting.
Proficient with EMR systems (we use HINT All in One).
Excellent communication skills with the ability to build rapport and trust with patients.
Organized, proactive, and team-oriented mindset.
Passion for preventive care and long-term patient relationships.
