- Complete, review, and submit MDS assessments in accordance with federal and state regulations
- Ensure PDPM accuracy and appropriate case mix index performance
- Monitor clinical documentation to support accurate coding and reimbursement
- Coordinate and participate in interdisciplinary care plan meetings
- Track assessment schedules and ensure timely submissions
- Collaborate with nursing leadership to identify documentation improvement opportunities
- Support regulatory survey preparedness related to MDS and reimbursement
- Educate clinical staff on documentation standards impacting reimbursement and compliance
- Active, unencumbered Registered Nurse license in CO
- Minimum two years of clinical experience in Skilled Nursing or Long Term Care
- Working knowledge of MDS 3.0 and PDPM reimbursement methodology
- Strong understanding of Medicare and Medicaid regulations
- Prior MDS coordination experience preferred
- Strong analytical skills and attention to documentation detail
- Ability to collaborate effectively with interdisciplinary teams
