    Lifesteps, Inc. is excited to add a new service in the Western PA area. We are creating a team of staff to support new moms and their child through pregnancy and their first 18 months of life. We are immediately hiring in locations throughout Pittsburgh, PA .

    Position Summary:

    The Home-based Maternal Health Specialist works to assist mothers in scheduling and attending post-partum and well-baby appointments; assess infant and mother care and home supports; monitor infant developmental stages; and offer guidance, education and referrals to parents as appropriate to achieve positive support outcomes through home visiting.

    Monday through Friday, Vehicle Provided

    ESSENTIAL FUNCTIONS – JOB SPECIFIC:

  • Create and maintain respectful partnerships with families.
  • Prepare home visiting plans in conjunction with parents.
  • Drive to family home and deliver health, educational, nutritional, child growth and development information to parents including support in scheduling postpartum and well-baby follow-ups through home visits.
  • Work with parent(s)/guardian(s) to develop an individual plan for his/her child to include goal setting based on identified needs.
  • Complete member engagement and home visits in a timely manner following Managed Care Organizations (MCO) and Lifesteps guidelines.
  • Assist and support families in locating resources and scheduling appointments regarding social determinants of health referrals and MCO member services available.
  • Provide family-centered services that facilitate child development, support parental roles and promote self-sufficiency.
  • Provide support in parent and child activities that encourage developmentally appropriate activities for the changing needs of infants.
  • Record Keeping and Documentation

  • Submit reports as required to supervisor.
  • Maintain child records, including, but not limited to screenings, immunizations, scheduled appointments, follow-ups, educational material given, along with referrals.
  • Gather and maintain individual, family and socialization data for documentation, ongoing assessment, evaluation, and recordkeeping for successful individual and program planning.
  • Complete accurate data collecting and information through multiple reporting tools according to MCO and Lifesteps time frames and guidelines.

  • Communicate with families in their spoken language or make accommodations and work closely with an interpreter.
  • Perform first aid and CPR as certified.
  • QUALIFICATIONS:

    Required:

  • Associate Degree in Early Childhood Education, Elementary Education, Child Development, Family Studies, Psychology or a Human Service field OR a High School Diploma/GED and a Child Development Associate (CDA) credential.
  • Demonstrate competency to plan and implement home-based learning experiences that ensure effective implementation of the home visiting curriculum and promote children's progress across the standards described in the evidence-based tool provided, included for children with disabilities and dual language learners, as appropriate, and building respectful, culturally responsive, and trusting relationships with families.
  • Knowledge in infant/toddler development/education/health/nutrition/safety as well as family dynamics and adult learning principles.
  • Knowledge of community resources and the skills to link families with the appropriate agencies and services.
  • Must be able to secure, within 90 days of hire, and maintain the basic first aid and pediatric CPR certification.
  • Demonstrated ability to complete basic computer tasks or Proficient with Microsoft Office Suite.
  • Current driver's license and the ability to travel for business utilizing personal vehicle Preferred :
  • Bachelor's degree or higher in Early Childhood Education, Early Child Development, Children Services (or related field)
  • 1 year experience Early Education, Early Childhood Development, Children Services, Social Work, Women's Health or related field
  • Bilingual English/Spanish speaking
