    SUMMARY

    Provides Palm Beach PACE participants with nursing assistance, transporting patients to scheduled clinics, and support as needed in a facility setting (Assisted Living Facility (ALF) or Long-Term Care (LTC)).

    PRIMARY FUNCTIONS

    • Client Care: Provide support to facility staff with client care. This includes, but is not limited to, listening, and responding to concerns and questions, and completing all necessary and patient care, including toileting, vital signs, repositioning, transferring, bathing & dressing. Reporting all concerns and changes to appropriate nursing
    • Nutrition of Client: Involves assisting residents by providing them with all their nutritional needs, including, but not limited to assistance with feeding, cueing, and encouraging hydration as tolerated.
    • Activities and Engagement: Assisting participants with appropriate Performing other duties as assigned.
    • Professionalism: Involves maintaining a neat and clean appearance, the willingness to learn new concepts, accepting accountability for job performance, the ability to manage time effectively, and promoting compassion and understanding for the client's needs/wants.
    • Attendance and Punctuality: Involves coming to work when assigned, checking the timecard, checking the schedule, and reporting for the assignment promptly.
    • Safety for Self and Residents: Involves personal safety in all work, being alert, and watching out for the safety of the client as well as following all safety protocols established by Facility management.
    • Infection Control: Following all infection control guidelines and protocols established by PACE and Facility
    • Customer Service and Communication: Involves speaking with clients, families, and staff; with tact, diplomacy, and strong listening.
    • Documentation: Complete required documentation after the completion of care.
    • Time Management and organization: Ability to manage assigned tasks and care in an organized manner.
    • Continuing Education: Involves participating in all mandatory in services, and compliance with State of Florida Board of Nursing continuing education for the Certified Nursing Assistant.

    SUPERVISORY RESPONSIBILITIES

    This position does not have supervisory responsibilities.

    QUALIFICATIONS (EDUCATION and/or EXPERIENCE)

    Certified Nursing Assistant with a High school diploma or general education degree (GED); with 1 year of CNA experience.

    LANGUAGE SKILLS

    Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of an organization.

    REASONING ABILITY

    Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

    CERTIFICATES, LICENSES, REGISTRATIONS

    • Certified Nursing Assistant License certificate issued by the State of Florida
    • High School Diploma or equivalent,
    • Valid Driver's License

    PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to stand and walk; use hands to finger, handle, or feel; reach with hands and arms; talk or hear, taste and smell; occasionally required to climb or balance, stoop, kneel, crouch, or crawl; and will rarely sit. The employee must regularly lift up to 25 pounds and occasionally lift up to 50 pounds. Must be able to lift with the use of lift devices or assistance for residents with weight over 100 pounds.

    Work Environment

    While performing the duties of this job, the employee is frequently exposed to fumes or airborne particles and toxic or caustic chemicals. The employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, risk of electrical shock, and risk of radiation. The noise level in the work environment is usually moderate.
