    Field Physical Therapy Assistant for Home Health Visits

    Pay Rate - Starting Visit Rates at $ per routine plus mileage

    $2500 Sign On Bonus


    Activa Home Health (an AccordCare Company) is a licensed Medicare certified home health agency seeking an experienced full-time Field Physical Therapy Assistants.


    POSITION SUMMARY


    Responsible to the Director of Professional Services, Clinical Manager to provide medically prescribed, goal oriented therapy in accordance with the Physician Plan of Care, Company Policies and professional Standards of Practice.

    Supervises personnel to deliver care and coordinates care delivery.


    JOB RESPONSIBILITIES
    Participates in the development of patient physical therapy plans of care under the supervision of the Physical Therapist. Makes visits to the patients and communicates with the physician as needed.

    Follows the Plan of Care and documents the care and submits electronic documentation in an accurate and timely manner to update and maintain the patient medical records.

    Provides education and instruction to team members, patients and their families.

    Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the patient and agency and family, exchange information and problem solve, and receive staff support and education.

    Applies the policies and procedures of the agency and the rules and regulations of Federal and State Regulatory agencies and Accreditation Commission for Health Care in providing care.

    Coordinates patient/family plan of care and ensures the care plan is reviewed at every team meeting. Assesses patient's level of care, needs and requirements. Completes electronic paperwork in an accurate and complete manner and adheres to agency policy for timely submission.

    REQUIRED EXPERIENCE, EDUCATION, SKILLS & ABILITIES


    • Currently licensed as a Physical Therapy Assistant (PTA) in this state and in good standing with the Board in which he/she will practice.
    • Knowledge of principles, concepts and methodology of physical therapy.
    • Ability to work well with an interdisciplinary team.
    • Ability to develop and maintain rapport with the Patient and family.
    • Excellent written and verbal communication skills.
    • Demonstrate problem solving and decision making abilities.

    REQUIRED CREDENTIALS

    • Current CPR
    • Level 2 Background screening
    • Recent negative TB / CXR
