    We are seeking a compassionate primary care provider to join our team in La Junta, CO. This is an exciting opportunity for a dedicated professional who values connection and continuity of care.

    About the Role



    The successful candidate will conduct thorough physical exams and health assessments, diagnose and treat acute and chronic medical conditions, order diagnostic tests as necessary, develop individualized treatment plans, prescribe medications as appropriate under state regulations, maintain accurate documentation in electronic health records (EHR), educate patients on diagnoses and treatment options.



    Key Qualifications:



    • A graduate degree from an accredited Nurse Practitioner or Physician Assistant program is required; Active Colorado state license & relevant specialty certification preferred;

    • A minimum 2-4 years clinical experience preferably in primary care or pain management is required;

    • An excellent communicator with strong interpersonal skills & compassion for delivering exceptional patient-focused service is essential;
