    The Service Coordinator is responsible for enablingresidents of Fowler Christian Apartments to continue to live independently atthe highest possible level of spiritual, emotional and physical functioning.

    The Service coordinator is responsible for ensuring residents ofour Senior living Affordable Community are aware of and connected to the necessary anddesired support service available.


    Some key responsibilities include:

    • Maintainand provide oversight and awareness of residents physical and mental condition.
    • Research,develop, and maintain a network of service providers and eligibilityrequirements. Establish and maintain a service directory of local communityresources.
    • Providegeneral case management for residents and provide appropriate referrals toservice providers/community agencies as needed
    • Identifyand assess residents who are at risk. Recognize when residents needhigher level of care to maintain or improve their safety and health. Assistresidents and families in locating and assessing an appropriate facility.
    • Maintainan individual file for each resident documenting all incidents related to theresidents condition/situation.
    • Respondto activated resident emergency calls during regular office hours.
    • Conduct/assistas needed annual apartment inspections with maintenance to assess residentsliving environment.
    • Maintainand supervise the planning, scheduling and performance of activities for theresidents as needed. Provide social opportunities that assist residents inbuilding informal support networks between residents, family and friends.
    • Actas ex-officio member of resident council; attend meetings, and accept reports fromwing- reps and monitors.
    • Visit withand follow-up on residents in the hospital. Coordinate hospital reports withstaff, monitors and wing-reps.
    • Respond toinquiries concerning eligibility for HUD and HUD/STAR+ paired Assisted Livingfacility.
    • Maintaincurrent knowledge of HUD, STAR+ and Assisted Living regulations.
    • Educated orschedule training on service availability, application procedures, residentsrights, aging in place, sensitivity to the aging process and other topics asappropriate to staff, families and the community.
    • Organization,implementation and facilitation of residents volunteer activities within thecommunity; coordinate and assist with outside volunteer activities and groups.
    • Performother duties as assigned.
    Qualifications/Requirements:


    • Graduateof an accredited university with a degree in Social Work, Gerontology,Psychology, Sociology, Counseling or related field experience.
    • Verifiableexperience in a social service delivery system; minimum of 10 years.
    • Experiencein working with the elderly and/or disabled; knowledge of the aging process andissues.
    • Experiencein the successful development and execution of a senior activity program.
    • Mustpossess strong interpersonal skill and the capacity to relate to a variety ofpeople and personalities.
    • Mustpossess strong analytic skills and be able to form comprehensive solutions tothe situation you have analyzed.
    • Mustposses strong organizational, record keeping, and time-management skills, andbe able to work independently and as part of a team, with minimal supervision.
    • Musthave working knowledge of personal computers and ability to learn variousprograms/applications.
    • Mustattend weekly staff meetings, providing updated information from thedepartment.
    • Possess avalid drivers license and able to operate a motor vehicle.
    • Ability towork flexible hours and be on call.
    • Ability tocomply with residents rights, all regulatory requirements, and all companypolicies and procedures as stated now and in the future.
    13.. Must beable to read, write, speak and understand the English language.

    • Mustpossess leadership ability and willingness to work harmoniously with otherpersonnel.
    • Must havepatience, tack, cheerful disposition and enthusiasm, as well as be willing tohandle residents based on whatever maturity level in which they are currentlyfunction.
    • Bilingual is a plus
