    Overture Home Care is currently seeking a candidate for a case management position. We provide care for seniors in and around the Fort Worth area, including Aledo / Weatherford, Hurst, North Richland Hills, Alliance, and more.

    Overture Home Care offers RN-delegated home care and companion services to seniors and other individuals requiring care. The company's highly-trained caregivers are overseen by a staff RN, delivering a variety of services to enable their clients to live independently, safely and with dignity.

    We are looking for a Case manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client's health and human services needed. A certified case manager is a plus.

    Other duties of the Case Manager will be:

    • Ensuring the Core Values of Patience, Dignity, and Discretion are adhered to at all times.
    • Scheduling and ensuring that the agency is adequately staffed to provide quality care to clients.
    • Responsible for coordinating the client's visits and communicating with the Recruiting and Staffing Coordinator.
    • Responsible for completing on-site home admission assessments for new clients.
    • Responsible for developing the Care Plan for coordination of care for all clients.
    • Functions as a liaison between the client and the caregiver, actively eliciting feedback from the client.
    • Responsible for completing competency assessments and validations for caregivers
    • Maintains records to support and validate caregiver competency according to state rules and regulations.
    • Ensures the provision of in-service/staff development programs that contribute to meeting agency objectives and standards and comply with all federal, state, and local regulations.
    • Maintains accurate records of in-service education and additional education
    • programs for staff.
    • Participates in on-Boarding presentations as needed.
    • Establishes and facilitates effective employee/client relationships.
    • Maintain the confidentiality of all pertinent client care information to ensure client's rights are protected.
    • Assist in performing caregiver assignments when there are shortages (appropriate training needed).
    • Responsible for answering the staffing phone to adjust schedule due to unforeseen events.
    • Be on-call as needed.
    Requirements
    • Previous experience in a home care or home health setting
    • Previous scheduling experience as well as case management experience
    • Must have a High School Diploma or GED
    • Ability to pass a through background screening and reference check
    • Knowledge of Wellsky scheduling software a plus

    $50000 per year
