Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections with precision, including IV infusions and catheterizations, adhering strictly to safety protocols and HIPAA regulations.
- Support complex medical procedures such as wound suturing, tube feeding, and ventilator management while maintaining aseptic techniques.
- Document all patient interactions accurately using EMR (Electronic Medical Record) systems like Cerner or Athenahealth, ensuring compliance with medical documentation standards.
- Assist with diagnostic evaluations including specimen collection/processing and medical imaging preparations; support triage efforts for urgent and emergency cases.
- Manage patient care in specialized areas such as dementia care, Alzheimer's care, hospice, or memory care units, providing compassionate support tailored to individual needs.
- Collaborate with multidisciplinary teams including physicians, case managers, and therapists to develop discharge plans and coordinate post-acute or home & community care services.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and pharmacology principles.
- Hands-on experience with EMR/EHR systems like Cerner or Athenahealth; familiarity with medical documentation standards such as NCQA or HIPAA compliance.
- Competency in performing IV insertions, phlebotomy, venipuncture, catheter care, and basic life support (BLS).
- Critical care experience preferred—especially within emergency medicine, ICU settings, or trauma centers—along with skills in airway management and acute pain management.
- Ability to work effectively with individuals with disabilities or developmental disabilities; experience in geriatrics or pediatric settings is advantageous.
- Excellent patient assessment skills combined with the ability to handle high-pressure situations calmly and efficiently.
