    Job DescriptionPosition Purpose:

    The Mental Health Rehabilitation Specialist will be an integral part of a field-based team responsible for delivering comprehensive mental health services to clients enrolled in a DMH contracted program at JWCH Institute, Inc. This role involves providing support to adults facing serious mental health illness and substance use disorders, encompassing comprehensive mental health rehabilitation services, targeted case management, and crisis intervention.

    Principal Responsibilities:

    1. Conduct field-based work (e.g., County Jail and community-based settings).
    2. Conduct screening assessments for all clients.
    3. Develop comprehensive care plans in collaboration with therapists for all clients and monitor them continuously.
    4. Work collaboratively with team members.
    5. Provide appropriate referrals for services, including medical care, housing, food, transportation, benefits counseling, etc.
    6. Offer individual and group rehabilitation services to clients enrolled in the program.
    7. Provide ongoing case management services, including linkage and referral to both internal programs and external resources.
    8. Maintain productivity requirements to meet contract compliance.
    9. Complete all necessary charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation.
    10. Participate in structured and "as-needed" case conferences.
    11. Collect necessary data for reporting purposes, as requested.
    12. Refer clients to appropriate services.
    13. Participate in agency and community meetings.
    14. Participate in after-hours on-call rotation as needed and assigned by the team lead and/or director.
    15. Conduct screenings for all service requests promptly (as defined by DMH Access to Care standards).
    16. Accompany clients to appointments as necessary.
    17. Support clients in obtaining housing and benefits as needed.
    18. Assist clients in building skills to reduce impairment from medical and mental health illnesses.
    19. Coordinate outreach activities both within and outside the clinic.
    20. Perform other assigned duties.

    Requirements:

    1. BA degree and four years experience in a mental health setting (physical restoration, social adjustment, or vocational adjustment). Two years of graduate education may be substituted for years of experience or two years of post-AA clinical experience may be substituted for educational experience.
    2. Two years of experience in related field services with homeless and in a health care setting.
    3. Good verbal and written communication skills
    4. Experience with Electronic Health Record keeping

    Preferred:

    1. Experience in outpatient clinic setting working with low income, incarcerated or homeless individuals dealing with mental illness and addiction.
    2. Bilingual (English/Spanish) ability.
    3. Experience working in a DMH funded program.
