Job summary

Evaluates physician medication orders for appropriateness of drug dosage potential interactions Maintains patient database to include appropriate current and past medical history clinical and laboratory data and concurrent medication Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered evaluated and discussed with appropriate members of the healthcare team Obtains as necessary and maintains all required documentation in accordance with federal state regulatory agencies and policy Informs the Pharmacist in Charge of medication related errors Performs other duties as assigned

• Bachelor's Degree in Pharmacy is required
• Knowledge of principals related to pharmaceutical preparation practices Candidates need to be fully licensed in PA NJ DE or willing to get licensed in all three states Two years' experience as a pharmacist long-term care pharmacy preferred Strong leadership skills Detail oriented goal oriented team player oral communication A combination of comprehensive therapeutic knowledge problem-solving skills judgment is necessary Ability to work cooperatively with others comprehension of the current standards of pharmacy practice understanding challenges professional pharmacy practice Ability master computer/software applications necessary completing job tasks meet deadlines Excellent verbal communication skills
