    Job responsibilities:
    Document assessments, plans, and communications in the medical record.

    Coordinate discharge planning activities and communicate plans with patients and care teams.

    Maintain confidentiality and follow established documentation standards.

    Work 45 hours per week with 9-hour shifts.
