    Description

    Registered Nurse - Home Health Care

    Our healthcare organization is seeking a skilled Registered Nurse to join our team providing direct nursing care to patients and families in their homes. The ideal candidate is a strong communicator with a caring demeanor and is able to provide a high level of patient care while coordinating home healthcare activities. This position reports directly to the Home Health Care department.

    • Job Type: Full Time hours per week)
    • Shift: Days
    • Sign-On Bonus and Relocation Reimbursement: Available for external candidates
    • Qualifications:
      • Education: Nationally accredited nursing graduate
      • Credentials: Current CPR for Healthcare/BLS and RN license in NM or as allowed by reciprocal agreement by NM
      • Experience: At least 1 year of directly related experience

    Responsibilities:

    • Assess patient's physical, psychological, social and environmental status; evaluate patient's response to approved plan of treatment; modify treatment plan, as necessary
    • Provide nursing care and coordination of ancillary services to achieve treatment plan goals; supervise ancillary nursing staff in patient care provision
    • Administer medications and perform treatments for assigned patients in accordance with medical and nursing protocols
    • Report pertinent observations and reaction regarding changes in patient status to the appropriate team person
    • Provide follow-up to ensure problem resolution; record observations and patient care activities
    • Interpret agency philosophy and policies to patients, families, and other groups; utilize agency and community resources to the benefit of the client
    • Establish and maintain good rapport and effective working relationships with patients, visitors, physicians and hospital employees
    • Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings, and workshops
    • Follow patient safety-related policies, procedures, and protocols
    • Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
    • Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
    • Report potential or actual patient safety concerns, medical errors, and/or near misses in a timely manner
    • Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
