Summary:

The Habilitation Technician will work directly with Individuals with Intellectual and Developmental Disabilities and staff to promote the wellbeing of the client.

They are responsible for direct, basic, and supportive care of Individuals within a program by performing their required duties and responsibilities.

Essential Duties and Responsibilities:
Provides training, health monitoring, assistance with activities of daily living, and transportation for adults with Intellectual and Developmental Disabilities.
Provides support and utilizes approved intervention methods to assist Individuals who may be experiencing maladaptive behaviors.
Serves as a member of the Individual's team and facilitates advocacy.

Maintains daily logs and records of Individual's care, condition, progress, or concerns to report and discuss through the chain of command.

Monitors Individual's safety including physical, medical, and mental health needs.
Documents Individual's progress towards defined goals and instances of maladaptive behaviors consistent with the Individual's program plan.

Assists with cooking, cleaning, and other general chores necessary to maintain sanitary conditions and general upkeep of assigned work site.

Promoting integration into the community and facilitating outings by providing transportation and assistance.
Complies with federal, state, and company policies, procedures, and regulations.
Maintains confidentiality of all information and complies with HIPAA regulations.

Demonstrates knowledge and support of Westbrook Health Services' mission, vision, values, policies and procedures, operating instructions, confidentiality standards, and code of ethical behavior.

Other duties may be assigned to meet business requirements.


Qualification Requirements:
Works well in a team environment
Work with others in a professional, cooperative, ethical, respectful, and effective manner.
Has a valid driver's license and current auto insurance.
Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen.

Education and/or Experience:
Successful completion of Title XIX waiver and other Westbrook Health Services training as applicable (provided).

Physical Requirements The employee is frequently required to walk; use hands to finger, handle, or

feel; and reach forward with hands and arms. The employee is occasionally required to sit and stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 50 pounds.
