    Job Description

    We are seeking a skilled Nurse Assessment Coordinator to join our team at Capital Health Care Network. This role is responsible for coordinating the assessment process for residents, ensuring that their needs are met and their care plans are up-to-date.

    About the Role

    • Develop comprehensive assessments of nursing needs for each resident, utilizing MDS 3.0 and RAI Process
    • Cordination with interdisciplinary teams to develop written plans of care, setting goals and objectives for each resident's medical, nursing, and psychosocial needs
    • Ensure timely completion of assessments and plan updates, including quarterly and annual reviews

    Responsibilities

    1. Preliminary and Comprehensive Assessments: Develop assessments of nursing needs for each resident, using MDS 3.0 and RAI Process
    2. MDS 3.0 Resident Interviews: Ensure timely completion of MDS 3.0 resident interviews, with staff interviews completed if necessary
    3. Care Plans: Coordinate development of written plans of care, identifying problems/needs, care to be given, goals, and responsible professionals
    4. Timely Submission: Ensure timely electronic submission of all required MDS 3.0 assessments and tracking
    5. Care Area Assessments (CAAs): Review CAAs with related CATs, complete narrative summaries, and verify signatures
    6. Dissemination of Materials: Disseminate new or updated materials involving MDS 3.0 and RAI Process
    7. Family Participation: Create opportunities for family participation in care planning
    8. Case Mix Scores: Communicate with Business Office Manager and Administrator on case mix scores and reimbursement impact
    9. IDT Meetings: Lead daily PPS and Utilization Review Meeting, weekly Triple Check IDT Meeting, and weekly Case Mix Review Meeting

    Requirements

    • Licensure as an LPN or RN
    • Experience in long-term care or similar settings
    • Strong communication and interpersonal skills
    • Ability to work effectively in an interdisciplinary team
    • Knowledge of MDS 3.0 and RAI Process

    Benefits
