    System Case Management Role at SSM Health St. Anthony Hospital

    About the Position:

    This role is responsible for assessing, developing, planning, implementing, and evaluating patients' discharge planning needs to ensure a smooth transition to the next level of care.

    Main Responsibilities:

    • Ensure compliance with Condition of Participation (CoP) discharge planning guidelines.
    • Identify and escalate gaps/barriers related to inpatient and observation length of stay.
    • Assess patients at high risk for readmission, identify root causes, develop plans, and collaborate with multidisciplinary teams for discharge planning.
    • Participate in quality improvement initiatives and contribute to standard work opportunities.
    • May also be responsible for utilization management, ED case management, admission processes, and liaison work with post-acute providers.

    Requirements:

    • Graduate of an accredited nursing school or equivalent education for licensure.
    • Three years of acute care experience.

    Physical Requirements:

    • Frequent lifting, carrying, pushing, and pulling objects weighing 0-25 lbs.
    • Frequent sitting, standing, walking, reaching, and repetitive movements.
    • Frequent use of vision and depth perception for near and far distances.
    • Frequent use of hearing and speech for oral communication.
    • Occasional bending, stooping, kneeling, squatting, twisting, and gripping.

    Licenses and Certifications:

    • Registered Professional Nurse (RN) license issued by the state of employment (Illinois, Missouri, Oklahoma, or Wisconsin).

    Work Environment:

    This position is based in Oklahoma City at SSM Health St. Anthony Hospital, with regular working hours from 8am to 4:30pm, Monday through Friday.
