- Transitions of care Pharmacist supports high-risk, medically-complex patient with medication-related needs AS THEY TRANSITION from an inpatient hospital stay to their homes and communities.
- Pharmacist will exam medication-related problems to decrease possibility of safety complications and readmissions.
- Pharmacist-led medication interventions and medication education
- TOC Pharmacy program with clinical pharmacists who had experience working in safety-net clinical settings and/or serving vulnerable patient populations.
- Active California Pharmacist license required
- 1+ years of Pharmacist experience
- Must have Pharm D
- Must be 18+ years old
- Background and Drug Screening required
- Health Screening required
- Must be able to provide proof of eligibility to work in the US
