    Job Description

    1. Direct Patient Care
    2. Verification of chemotherapy regimen as ordered through the electronic medical record
    3. Coordinate dispensation of chemotherapy and supportive care regimen
    4. Optimizing medication therapies to prevent and reduce medication errors
    5. Therapeutic drug monitoring for appropriateness of use, dose, dosage form, and drug interactions
    6. Provide pharmacokinetic consultations when requested
    7. Provide patients with counseling as applicable
    8. Provide medication information to interdisciplinary Oncology teams
    9. Develop medication guidelines, order sets, and protocols in collaboration with clinical staff
    10. Pursue opportunities for optimization of Electronic Medical Record
    11. Document pharmacotherapy interventions within the Electronic Medical Record
    12. Education responsibilities:
    13. Provide patient specific education on chemotherapeutic regimens as needed
    14. Facilitate, develop, and implement lectures to pharmacy staff in oncology related topics
    15. Provide in-services to medical staff on oncology related pharmacological topics
    16. Provide in-services to nursing staff on oncology related pharmacological topics
    17. Development of guidelines and protocols related to oncology care.
    18. Departmental responsibilities
    19. Ensure safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols
    20. Facilitate the development and implementation of treatment guidelines, protocols, formulary changes as approved by appropriate committees. Ensure guidelines & protocols match national standards and include clinically appropriate treatment standards, while respecting the cost structure of the institution
    21. Coordinate the selection and safe use of alternative medication in response to medication shortages
    22. Provide clinical consultation and clarification to practitioners
    23. Support the pharmacy departments clinical endeavors
    24. Review the use of targeted medication for compliance with pharmacoeconomic programs
    25. Suggest appropriate, cost effective therapeutic alternatives to medical staff
    26. Monitor drug therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, adverse drug reactions, appropriateness of drug and dose, and therapeutic duplication
    27. Help reduce or limit cost of medications utilized in the treatment of oncology patients through review of each specific patient medication regimen
    28. Provide accurate, adequate, and timely drug information to professional staff
    29. Prepare and review drug use evaluation criteria, adverse drug reaction reports, and pharmacy interventions summary to various committees
    30. Participate in hospital education programs
    31. Coordinate policies and procedures for clinical activities (i.e. therapeutic interchange, unacceptable abbreviations, look alike-sound alike drugs, medication events reporting etc.)
    32. Participate in quality improvement activities of the department
    33. Collect, analyze, and maintain data for all documented interventions
    34. Establish productive, collaborative relationships among the multidisciplinary healthcare team
    35. Sustain professional competence and promote development through participation in in-services, workshops, seminars, and professional organizations
    36. Precept and mentor pharmacy students for local Colleges of Pharmacy
    37. Train pharmacists and pharmacy students to participate in drug use evaluation
    38. Facilitate discharge/transfer planning and transitions of care related to medication use
    39. Complete special projects as assigned by Pharmacy Leadership
    40. Participate in Pharmacy and Therapeutics committee through facilitation of drug formulary additions

    Minimum Qualifications

    Required Qualifications

    1. Possession of a valid NY State license to practice pharmacy.
    2. Two years of post-licensure experience in Oncology Clinical Pharmacy Practice; or
    3. Completion of a post graduate specialty residency or fellowship program in Oncology Pharmacy Practice

    Preferred Qualifications

    1. Current Pharmacy Board Certification in Oncology (BCOP)
